My Aged Care Support Program

Free Local Support from a Trained Peer Champion on Fleurieu Peninsula, Yorke Peninsula and Murray Mallee

Are you aged 65 or over? Are you, a friend or relative struggling to navigate the My Aged Care Website or phone service?

If the answer to either of these is yes, this free support service, funded by Country SA Primary Health Network (PHN) and facilitated by COTA SA, could be for you. One of our trained local Aged Care Peer Champions can assist you to access the information you need and help you navigate the My Aged Care system. Free one on one support is available over the telephone initially, and your local Peer Champion can meet with you face to face if required.

Come Along to a Free community Information Session

In each region a community information session for the public, will include presentations on Commonwealth Home Support Program and Home Care Packages. The sessions will also present local service providers an opportunity to share what services they offer to older people in the local community. The Peer Champions will be in attendance and be available to book or provide support sessions. Click the links below for details of the session in your local area:

About the program

Between July 2017 and 30 March 2018, the project will be trialling the delivery of practical one-on-one peer support with understanding and navigation of the My Aged Care gateway to older people in:

• Kadina
• Victor Harbor
• Murray Bridge and their surrounding locations

Findings from the trial will support the development and delivery of future support projects throughout South Australia and nationally.

Why this program was developed

While there has been an increase in the understanding of the aged care system since the reforms in 2015, there is still an ongoing and significant need for support and assistance with how the aged care system operates, including the differing levels of care and support available locally. The project aims to deliver the support needed through trained Aged Care Peer Champions. The Peer Champions will assist older people still living independently to access the information they need and help them navigate the My Aged Care system.
